Elections: Voter Registration

To register to vote online, please visit the Secretary of State's website.

To mail in a voter registration card, please complete the form below and return it to Town Clerk.

A citizen can register at Town Hall during normal business hours or at a voter registration session prior to the election, by mail, at the Registry of Motor Vehicles or at a special registration session. You can pick up a voter registration card at the Town Clerk's Office. 

The following should be completed:

1. Name (last name, first name, middle initial)
2. Former name if applicable
3. Address
4. Address where you receive mail (if different)
5. Date of birth
6. Identification number - last four digits of the social security number or the license number is mandatory
7. Telephone number is optional
8. Party enrollment
9. Address where you were last registered to vote
10. Date
11. Signature
